How to Join

If you are an employee of The London Ambulance Service you may join the Benevolent Fund at any time by downloading and printing both of the forms below. Both the Joining Deduction form and the Benificiary Form must be completed in full, including witness and posted to:

To update an existing members benificiary details, only the completed and witnessed benificiary form is required.
